We compare the lowest recorded price per clinic, matching the report. Included and excluded require an explicit statement in that price’s description or note. Not stated means the record does not establish it; it does not mean an extra charge. Offering ICSI does not establish that it is included in the price.
Medication mentioned only for retrieval is not classified as general coverage. Even when marked included, confirm the drugs and dosage limits. Freezing and storage are recorded separately; check the evidence for the storage period. This analysis classifies the cited records and preserves their original access dates: it is not a fresh check of every price or a current quotation.
